Proclamation Request

Requests for proclamations to be presented at a Council meeting, an event, or for individual needs are submitted by staff to the Mayor for approval. All requests must be received 6-8 WEEKS in advance of the date needed. All fields are required. Incomplete forms will be returned to the sender. Additional attachments WILL NOT be accepted.
